This technique is known as radial forearm flap phalloplasty.

Talking about the arm scar after phalloplasty is not only about aesthetics. During surgery, more than just skin is taken. A complex set of tissues is also involved, including arterial supply, venous drainage, and nerves. The goal is not only to create volume but to reconstruct a viable, sensitive, and functional neophallus.

Why is the arm used for phalloplasty?

Among the different phalloplasty techniques, the radial forearm free flap remains one of the preferred options in many cases. It provides thin, pliable, well-vascularized tissue with anatomy that facilitates microsurgical reconstruction. This allows surgeons to create a penis with good shape, improve the viability of the transferred tissue, and offer options for urethral reconstruction and sensory recovery.

In other words, building a penis does not simply involve transferring skin from one area to another. The tissue must be able to survive in its new location, receive adequate blood supply, and connect to nerves to develop sensitivity. That is why, in this type of surgery, the arm can provide much more than just skin coverage.

Which arm is used for the flap? Usually, the non-dominant arm. If you are right-handed, surgery will typically be performed on the left forearm; if you are left-handed, on the right.

Why an Arm Scar Appears?

The scar appears because that area of the forearm becomes the donor site for the flap. Once the tissue is transferred to construct the neophallus, the remaining defect in the arm must be covered, usually with a skin graft. As a result, the scar may be visible and show differences in color, texture, or surface compared with the surrounding skin.

For this reason, when we talk about the arm scar in phalloplasty, we are not referring to a small mark on the skin, but rather the trace left by a complex reconstructive technique. That scar exists because tissue capable of providing vascularization and sensitivity to the reconstructed penis has been taken from that area.

Sensitivity and functionality in phalloplasty

In phalloplasty, the goal is not only aesthetic. Surgeons aim to reconstruct a penis with the best possible balance between shape, viability, sensitivity, urinary function, and erogenous function. However, not everything is achieved in a single surgical procedure.

At IM GENDER, we perform staged phalloplasty, with the aim of optimizing urinary and sexual function, aesthetics, and overall surgical safety.

When the forearm is used, one of its advantages is that it allows the inclusion of vascular and nerve structures suitable for microsurgical anastomoses. This is what helps the transferred tissue survive and, over time, develop some degree of sensitivity.

Is the arm scar from phalloplasty very visible?

Not all scars evolve in the same way. Factors such as the size of the flap, the integration of the skin graft, each person’s healing process, skin tone, postoperative care, and the time elapsed since surgery all influence the final appearance.

However, although the scar at the donor site may be aesthetically noticeable, functional impact on the arm in everyday life is usually minimal.

How the forearm scar evolves

During the first weeks and months, the priority is for the donor site to heal properly. At first, there may be swelling, redness, tightness, hypersensitivity, or irregularities on the skin surface.

Although the process can take weeks or even months, scars generally improve over time. For this reason, it is important to assess the final result after a sufficient healing period. Skin and graft evolution is gradual, and in some cases additional treatments may be suggested once the scar has stabilized.

Pre-surgery care

At IM GENDER, patients are advised to start laser or IPL hair removal on the non-dominant forearm at least three months to one year before surgery, in order to remove as much hair as possible from the area.

Postoperative care of the donor site

Once discharged from the hospital, the patient returns home with a bandage on the arm consisting of a plaster splint and a dressing covering the surgical area.

This dressing should be changed every 48 hours. During the first week, dressing changes should be performed by healthcare professionals. After that, they can be done at home, always following the instructions provided by the medical team.

It is also important not to wet the donor site until about 10 days after hospital discharge, and to keep the arm elevated above elbow level. For this reason, the use of a sling is usually recommended.

Once the donor site has healed properly, the next step is to use moisturizing and emollient creams to keep the skin in the best possible condition.

Later, once the wound has fully closed and healing allows it, measures to improve scar quality may be recommended. These may include moisturizing creams, scar massage, strict sun protection, and specific medical treatments depending on the skin type and the scar’s appearance.

Treatments to improve the arm scar after phalloplasty

The goal is not to eliminate the scar entirely, as that is not realistic, but to improve its appearance, smooth irregularities, reduce contrast, and help the person feel more comfortable with that area of their body. This is possible thanks to aesthetic treatments after phalloplasty.

The most appropriate treatment will always depend on a personalized medical recommendation, but it is common to combine several procedures to achieve optimal results.

Laser for scars

Laser treatment is one of the most effective options for improving the appearance of the forearm scar after phalloplasty. It can help smooth skin irregularities and enhance the overall appearance of the skin, always after an individual assessment.

Autologous fat or lipofilling

Lipofilling, also known as an autologous fat graft, can be used to improve depressions, volume irregularities, and certain aspects of tissue quality. It is one of the most interesting aesthetic refinement options for the arm after phalloplasty.

Biostimulation

Biostimulation with platelet-rich plasma (PRP) has been used in regenerative medicine as a complementary treatment for tissue repair and scar improvement. Each case must be evaluated individually to ensure a truly personalized approach.

Tattoos and micropigmentation

Not everyone wants to treat the scar to conceal it. Some people prefer to integrate it in a different way. In this context, tattoos and micropigmentation can be alternatives once the scar is mature and the professional team considers the skin ready.

In some cases, micropigmentation may be used to even out skin tones, soften contrasts, or reinterpret the scar from a more personal perspective.

The importance of talking about the scar with your doctor

When considering phalloplasty with a forearm flap, it is important to discuss openly and clearly what you expect from the procedure. This includes not only the result of the new phallus, but everything the technique involves: postoperative recovery, healing timelines, realistic expectations, and also the scar that will remain on the forearm.

In this sense, it is essential that the medical team clearly explains why that area is chosen, what it contributes in terms of vascularization and sensitivity, how the scar typically evolves, and what options may exist to improve it later.

Do you have more questions about the arm scar after phalloplasty?
