Vaginal depth is an individual parameter that depends on anatomy, the amount and quality of available skin, and the surgical technique used.

What Is Vaginal Depth?

Vaginal depth refers to the internal length of the vaginal canal, measured from the entrance to the end of the neovagina. A cisgender vagina typically measures between 7 and 10 centimeters at rest, although it is highly elastic, allowing it to adapt during penetrative sexual intercourse.

In the case of trans vaginas, this length can vary depending on the technique used and individual anatomy, but the most important factor is that the canal is functional. “We always explain to patients that the vagina is not a fixed-size cavity it is tissue that adapts. What matters most is comfort, the ability to dilate, and functional response,” notes Dr. Labanca.

Surgical Techniques and Their Relationship to Depth

Today, three main vaginoplasty techniques are commonly used. Each can provide adequate depth, and the choice depends on the patient’s anatomy and available tissues.

Penile Inversion Vaginoplasty

When there is sufficient penile and scrotal skin, the vaginal canal is created using these tissues. Penile inversion vaginoplasty is the most traditional technique and can provide good depth and sensitivity with a very natural aesthetic appearance, as long as the quantity and quality of skin are adequate.

Peritoneal Vaginoplasty

If the available skin is limited or not very elastic, the peritoneum, a thin membrane lining the abdominal cavity can be used. The initial portion of the canal is constructed with penile skin, while the deeper section is created using peritoneum. This provides natural lubrication, elasticity, and durability. Notably, although it requires an abdominal approach, it does not involve opening the intestines.

Colovaginoplasty

In cases where there is very little skin available, a segment of the sigmoid colon is used. This option provides a longer, self-lubricating vagina, although it requires a more complex abdominal surgery.

“As a rule of thumb: when there isn’t enough skin, we use the colon; when there is plenty, penile inversion; and when there’s an intermediate amount with good elasticity, the peritoneum.
