When a trans woman seeks genital affirmation surgery or vaginoplasty, the medical team must meticulously plan the operation, taking into account each woman’s needs, medical history, traits, and personal circumstances. Our team will assess the case by categorising it into one of the three main techniques outlined below to remove the penis and create a functional and aesthetical vagina.
- Penile inversion vaginoplasty: This method is employed when the size, thickness, length, and quality of the penis’s skin allow for the creation of a vaginal canal with adequate width and size for regular sexual intercourse.
- Colovaginoplasty: This vaginoplasty technique is utilised when the penis is small or has undergone significant circumcision before genital reconstruction. It is also carried out when penile development is limited due to hormonal inhibitors used during prepuberty. In these cases, the penile inversion technique is not advised, as the vaginal depth would be insufficient, preventing the trans woman from having satisfactory intercourse.
- Penile inversion vaginoplasty with graft: When a trans woman’s penis does not have the appropriate dimensions for a successful penile inversion but does not require a colovaginoplasty, it is recommended to opt for a vaginoplasty with a skin graft. This approach provides the proper depth and dimensions for a vagina without resorting to the rectosigmoid flap technique.
Preparation before trans and non-binary vaginoplasty
Prior to undergoing transgender vaginoplasty, it is essential to prepare adequately, as this is just as important as postoperative care.
Having a blood test at least three months before the vaginoplasty procedure is strongly recommended. Furthermore, you should maintain a healthy diet and make sure your bowels are clean. Additionally, it is advised to abstain from alcohol, tobacco, or drug consumption one month before the surgery.
At IM GENDER, we acknowledge the importance of preserving hormonal treatment consistency in trans women. Therefore, oestrogen is not usually ceased prior to the operation. In the case of anti-androgens, like cyproterone, spironolactone, and triptorelin, it has been shown that stopping them is not required. As a result, these medications can be taken up until the procedure. Full discontinuation will take place one or two weeks following the transgender vaginoplasty.
